In effort to help identify missing children, the Virginia Department of Social Services requires all parents to present the school with proof of their child’s identity and age.  In compliance with this regulation, MSL asks that you provide us with that requested information.  Proof of the child’s identity and age may include:

️ ☀Certified copy of Birth Certificate
️ ☀Birth Registration Card
️ ☀Notification of Birth
️ ☀Passport
 ️☀Placement agreement or other placement proof

In the case of adoption, a placement agreement or other proof of the child’s identity from a child placement agency is also acceptable.  We ask that you provide us with this information at the time of your child’s enrollment, before his or her first day.  Please understand we do not need to keep the original document.  We will transfer the information on to the proof of identity form.
